This is one end product of an adventure that just gets weirder and weirder the longer I'm DM, so it's a good thing I'm stopping soon. The monster described below was trapped in a tomb on the material plane. It came from the Far Realm via a portal and was prevented from returning by a permanent prismatic wall cast by a heroic wizard. At the moment it's waiting for the wall to come down, comforted by faint psychic emanations from the other side.

A large creature sits slumped against a wall, muttering and gibbering to itself. It resembles a troll, but has enormous wings like dismembered stained-glass windows and a mass of writhing tentacles where its mouth should be. Its figure is warped and twisted into a grotesque imitation of a troll's, but it is clear that this is not the only form it can take.
I used an advanced troll (advanced according to the rules for giants because I didn't want it to have character classes), the Half-Fey and Half-Illithid templates from the Fiend Folio, and the Pseudonatural template from Manual of the Planes. I improved its regeneration and natural armor largely to compensate for house rules (players add BA to damage rolls - jeez) but it's not unreasonable to say that stuff resulted from its thoroughly alien nature. I say the creature used to be a normal troll but somehow wound up in the Far Realm, and the bizarre energies there turned it into this monstrosity. It'll attack anything that comes too close, but it's not evil or anything, just totally psychotic.
